RELEASE/WAIVER Pre-registration required. All sales final. Cancellations may receive take-home kits upon request. If you cannot attend: We understand that things happen. Although your fee cannot be refunded, we have options for you that we hope you consider to be helpful and fair. 1. You may give or sell your spot to a friend or family member. Please notify us of their names and grade levels at art@yucandu.com or 314-963-4400. 2. You may opt for us to create a kit with your supplies. You may use the complete kit at home or in the open-studio at your convenience, where you can take as long as you need. There will be a $5/hour studio fee. This is not a drop off time. Notify us of your choices please and will hold your kit for up to 30 days. I hereby agree to indemnify and hold harmless Yucandu and its employees from and against any and all claims for personal injuries or damages of any kind arising from participating in Yucandu programs. Further, I authorize Yucandu staff to seek emergency medical help if becomes necessary. I realize every effort will be made by Yucandu to contact me in event of a medical emergency involving my child and I agree to indemnify and hold harmless Yucandu personnel in seeking medical care for my child. REFUND POLICY: All sales are final. PHOTO CONSENT: By your signature, you agree that Yucandu may use the above named student’s photograph in future promotion of its activities. OUTDOOR LUNCH CONSENT: I give permission for my child to leave premises for outdoor seating near Yucandu. I understand lunch time is supervised by one or more staff members.
